Important Warranty Information
Submission of this form constitutes a request for warranty review and does not constitute approval or confirmation
of warranty coverage.
Warranty coverage is subject to the terms, conditions, exclusions, maintenance requirements, and applicable warranty
periods contained in your contract and any applicable manufacturer warranties.
Some products and equipment are covered directly by the manufacturer and may require the homeowner or Staycation to
submit a claim through the manufacturer.
Staycation may request additional information, maintenance records, water chemistry records, photographs, or an
on-site inspection before determining whether an item qualifies for warranty coverage.
Issues resulting from normal wear and tear, improper maintenance, improper water chemistry, homeowner or third-party
modifications, environmental conditions, ground movement, drainage, weather events, misuse, or conditions outside
Staycation's scope of work may not be covered.
Site Visit & Diagnostic Charges
Warranty requests that require an on-site inspection, diagnostic service, or evaluation may be subject to a site visit or service charge. If, after evaluation, the reported concern is determined not to be covered under the applicable Staycation, manufacturer, or vendor warranty, the homeowner may be responsible for reasonable inspection, diagnostic, travel, service, or site visit charges assessed by Staycation, the manufacturer, and/or the applicable vendor or service provider. Any subsequent non-warranty repair or corrective work will be quoted separately and will require homeowner approval before work is performed.
Access Authorization
By submitting this request, you authorize Staycation and, when applicable, its employees, subcontractors, manufacturers, or authorized service providers to contact you regarding the request and coordinate access to the property when an inspection or repair is required

By submitting this form, I confirm that the information provided is accurate to the best of my knowledge and understand that submitting a warranty request does not guarantee warranty coverage. Any determination of coverage will be made after review of the applicable warranty terms and circumstances of the reported condition.
